[om] Univariate Polynomials

I wrote:
  I merged in the fixes posted by Bill, changed the review date so it's in
  the future not in the past:-) and checked it with the stylesheet used by
  the cd contribution form (cdfiles/contrib/xsl/cd.xsl) which picked up a
  couple of CDs used in examples but not listed in the CDUses entry,
  I added those.


Also, added the standard distribution licence as used on all the other
CDs in the collection.
