[om-a] New CD collection

The CD collection at http://www.openmath.org/cd has been updated.

It contains numerous fixes from earlier releases (With thanks to the
CD Reviewers).

The MathML and Meta CD Groups are now classed as official OpenMath
Society CDs, http://www.openmath.org/cdfiles/html/public/
