While we were producing that document I sketched out some XSLT to do the
transformation, it wasn't complete (and then both openmath CDs and
mathml changed slightly as they neared there respective completion
dates, so the stylesheets would need further modification) so I never
actually released the stylesheets as they were likely to be  more
confusing than helpful.

Michael Kolhase asked the same question a month or two ago, and I sent
him the files. He did some further work on them but I'm not sure of the
current state.
