[Om] links from CD pages

The cd.xsl stylesheet has been enhanced to make links in comments and
descriptions active.

See for example

http://www.openmath.org/cd/altenc.xhtml

where the links to openmath.org in the licence text comment and to
www.w3.org/Math in the symbol description are now marked up as <a
href=... in the XHTML.
