[Om] prefix syntax specified?

I believe there was once a lisp syntax (to go with the SGML syntax) in
pre-xml era, but the particular form of prefix syntax as used by the
CD rendering stylesheets with hyperlinked symbol names, I just made up.
It's not actually standardised or documented anywhere.
